This form must be filled out completely before we can accept any samples. Once this form has been filled out, please coordinate with Colleen Ludka to drop off samples.

The most critical aspect of sequencing on Illumina's Genome Analyzer is the quality and accurate quantitation of the libraries and failed sequencing runs due to problems with library quality & quantitation is something that BIOGEM wishes to avoid. Therefore we ask that you work with Illumina tech support 800-809-4566 and/or Elizabeth Boyer to accurately evaluate your libraries. This is most important the first time you attempt to make a library.

Label the tubes with all three pieces of information

Sample name-please keep the number of characters to a minimum. We prefer samples that are labeled numerically.
